    I resolved my issue. Not sure if it was a recent WordPress update that changed something or I simply hadn’t ever noticed the “Toolbar Toggle” icon that collapsed all the extra toolbars & buttons for the editor. It was at the end of the first row of buttons, no second row.
    Everything’s up and running now. Thanks!

    Same problem here. I am using sparkling (bootstrap theme). Confirmed the editor is TinyMCE. The buttons do not appear. There is no second row in the editor. A few other plugins have shortcode buttons that show up (in the first row), not this one. Any advice? The plugin is activated & up to date.

    UPDATE –
    I discovered that the problem was from another snippet of code that I got from the same WordPress page which recommended your plugin – there was a bug in my code related to ‘image_size_names_choose’ custom filter. Plugin works great now that my bug is fixed.

    ORIG: Same issue. Just installed the plugin today on a 1 week-old wordpress installation.
    My pre-existing media files actually do show in the standalone Media gallery, but not on the popup where you can Add Media/Set Featured image/Insert, etc. It shows a spinning loading icon forever. No images appear or are searchable. Any attempt via the Media Gallery to attach an image is also unsuccessful. Rolling back. 🙁
