I went in to make this change and saw that I already had the Duplicate Record Preference set to overwrite.

You have another record that exists with the other half of the info if I list participants including your “Facility ID”. There is no way to match these 2 records.
There’s a disconnect between the list search and the signup form. The “match field” (in this case facility id) isn’t getting passed to the signup so it’s creating a new record each time. Do you have any advice on how I can set this up differently?

Yep, same here. I’ve got about 100 sites that I can’t update Contact Form 7 on because of this bug. Hope it get’s fixed soon.
(It would be extra confusing to my clients to have this checkbox shortcode printed into their message, because of the way it’s being used – explaining to all of them to ignore it would be a big logistic hurdle.)

Any checkbox field used in a form gets it’s shortcode printed in the message body of the email if it is not used. (Just as you’ve described)
I am rolling back to a previous version of the plugin until this gets addressed. Too many of my clients would be confused by this bit of code in the email, especially since it’s mainly used to opt in to a newsletter subscription.
