But I resolved problem. I have some functions add_permastruct and add_rewrite_rule that make problem. Just delete these functions.
But in both cases .htaccess file was same, unchanged.
I have checked and mod_rewrite is running. But it’s work about 10 days ago and I didn’t change anything related to pages and permalinks. Does some plugin can make some changes to permalink or htaccess?
Pages can be opened just for Default permalink structure (http://mydomain.com/?p=123)
