I use the download section to upload the file, instead of use “insert download” button.
I have tried the method you suggested by using the image, it works perfectly. Meanwhile, I did a simple test and the results are shown below.
1. New page + new pdf file = Work
2. New page + old pdf file = Work
3. Old page + new pdf file = Not workingSo conclusion is the contact form issue, but I’m not sure what is the issue yet.
I have created a new form and new page. But now I have another issue, the download monitor is not working. I can’t upload the files. But that’s weird, it worked last week.
Amended. I can download PDF but can’t open the file. Get the error message as shown below.
“Adobe Reader could not open ‘xxx.pdf’ because it is either not a supported file type or because the file has been damaged (for example, it was sent as an email attachment and wasn’t correctly decoded).”

The download link worked previously. But after I upgraded the plugin, it takes to a blank page as well, with the following URL.
