The HTML did look something like that but I have copied and pasted what you have above just to be sure and it doesn’t appear to have made any difference.
I added the HTML in the settings of Events Manager under “Formatting”. In there is a ‘Single Event Page’ which has a placeholder of #BOOKINGFORM. I just added the HTML under that.
This puts the checkbox under the booking button – is this wrong??
The Mailchimp list asks for First Name, Last Name, Email Address and has a hidden field for the source URL
