I personally have not come across an online contact form that does what you need. But, be patient – someone else may know of something that can help you.
A lack of reply can be just that. You did not have an actual answer to my question but you still made this post saying that you haven’t come across the plug-in.
oh, damn, I just came across one the other day. I found it in the codex plugin directory. Have a browse through there.
I had a quick look through there but didn’t see anything. I shall have another, more thorough look through it.
