For anyone who got here, my problem was that the ID field for several of my WordPress tables lost the auto increment flag.

The first thing you noted (“These should work just like a tag for a post”) made me think… let’s just try adding a tag through WordPress. It failed. So looks like it’s not an issue with your plugin. 🙂
