Seems like it was as simple as updating and network activating the plugin. Once I did this the Sample forms appeared on the backend. I then added the form and it finally appeared in the form list.

Thanks for the quick reply.
Plugin is currently running version 1.0.334.
Plugin has not been network activated but activated on each individual sub site.
The multi-site is using sub directories rather than sub domains. Zero configurations have been made to the network from the WordPress defaults.
All has been taken care of with this. It was a problem with our hosting provider sending all emails from our domain emails to our domain emails.
Got in touch with the host and they fixed the issue.
