This plugin has not been upgraded for WordPress 3.1 yet.
You need to disable safe_mode in PHP on your server or unset the open_basedir.

Aaron, it sounds as though you want the list to be in a more table based layout. Currently the plugin does not offer this functionality. My apologies for the inconvenience.
You’ll need to add some CSS. For instance you may want to apply a command to the DIVs you wish to see on the same line: display:inline;
Styling is tough because themes and stylesheets all contain varying rules. You’ll need to get creative.

I’ve had a few others with the same issue. I believe it must be an incompatibility with some other WP plugins. I have not been able to replicate this issue on any of my servers.

You could write a script that when importing the videos creates cached versions of the thumbs.
