Time was of the essence, so I instead installed Version 2.4 of Contact Form 7. The problem was instantly solved.
I’m not sure what could have been the cause? The exact installation state at the time of the error was 2.4.1 Contact Form 7, WP 3.0.1, and ShareThis 4.0.4 and Really Simple CAPTCHA 1.2… using the “Default” theme, first enabling “Day and Name” Permalinks. At that point I created Form 1 (no customization) and added it to a Contact Page with no other content. That’s when I tested the site and found the Form did did not display.
