Ok, I know exactly what the problem is. The customer (website) in question only charges tax for customers who reside in Washington State.
If the ‘Standard’ tax rate ‘City’ field matches the city typed into the checkout form, taxes are calculated normally.
If the tax rate ‘City’ field does not match the city field on the form, tax is not applied. For instance, for the state of Washington, sales tax description for zip code 98266 is “WHATCOM COUNTY UNINCORP. P.T.B.A.”, so if a customer who resides in that zip code who lives in Maple Falls, WA checks out, tax is not applied.

Taxes were calculating perfectly for several months, but stopped working in the last couple weeks.
Hosted by Bluehost, no caching. The tax issue is easily replicated.
