I’m facing the same issue (number 2) that the Original Poster was having.

By Fallback Image, they are referring to the Default Image setting on the Get Images processor. The issue is that when the input has no image tag, it uses that Default Image. However, that image gets added to the Media library for each post that uses it. So 3 posts don’t have an image tag, 3 Default Images will be added to the Media Library, rather than just one.
