Well it’s rather embarrassing to find that something so simple could fix it, but it does, so thank you.
I’m sure that I don’t usually use the http:// for links – it normally seems to work fine without them. So I’m still not sure why, in these few cases, it doesn’t. But at least I now know how to fix the problem.

I’ve tried emptying the cache, and the problem remains. Even on older pages, if I go back to edit them, the favicon disappears once they’ve been updated.
