RESOLVED
I found there was a conflict with the plugin Divi Assistant.
I’ve not yet found which DA setting is causing it but I’ve overcome it by adding:
.forminator-label { display: block !important; opacity: 1 !important; visibility: visible !important; }

Thank you, I changed the way they are submitted from ‘Ajax’ to ‘Page reload’ and this seems to have resolved the issue in most cases

As an update for future visitors to this post; I’ve now de-activated the ‘Find Us’ plug-in (which integrates Google Maps with WordPress by Roland Rust), which was on that page, and it’s now working fine.
