This sounds like Google is still showing an old page title or description from the previous theme. First, check the page title and site tagline in Settings → General and also check the homepage settings in WordPress. If everything is correct there, Google may simply be showing an older cached version. After making sure the current title is correct, request Google to recrawl the homepage through Google Search Console. It can take some time before the updated title appears in search results.
