I got a similar issue but I am not sure if the causes are the same.
The date and time fields are displaying just like simple text input fields.
No calendar or time schedule to choose from.I got no error messages.
I tried to deactivate some plugins just to check if no conflicts were occuring.
Nothing happens.I am quite in a deadend here.

NB : The reservations form is displayed on every single page.
Would you have an idea for me in order to solve this issue ?
