That was it – I needed to click Renew Mailchimp Lists. My new fields have now appeared as buttons and I can add them to my form.
I’ve used this plugin several times before and I haven’t had this issue.

My previous reply was for the suggestion about replacing all code in the template file. Which I did and that worked – just displaying Test.

Then I realised I had somehow missed your other message about the plugin upgrade.
I have upgraded and that really does seem to be working now!

There definitely aren’t any widget-for-eventbrite template files in the child or parent theme.

I haven’t tried the health check plugin yet. I’ll have time to do that tomorrow morning and I’ll let you know how I get on.
