I get the same ‘security error’ on login – but only intermittently.

Potentially a great application, easy to understand and use, fits our requirements exactly – but at the moment too many problems to use on a live site.
Am also having problems with the Auto Responder.
1. When initial request for approval is lodged, the email acknowledgement is sent OK.
2. When the pending request is ‘approved’, the approval is completed OK but no confirmation email is received.
3. This happens using both settings; webserver default and smtp.
