1) It is possible to customize the plugin appearance by using the different attributes and by separating each attribute with space. Here is the example for the same.

3) It is possible to show the FAQ on the pages also by using the FAQ shortcode.
4) You can create as many categories as you want.

Regarding the query, you can remove the subtotal field from the print-content.php file which is located at \plugins\woocommerce-delivery-notes\templates\print-order\print-content.php.
