Having trouble with the plugin after much the same fashion. I used it two days ago, came in and updated the shipping plugin and Jetpack both, and now the shipping button is not available for an existing order.
If I add a new order, however, the shipping button is available. Any suggestions? Thanks.
Quick correction:
On a bet with myself, I changed the table prefix back to wp_FPB, and now I can log back in. Though I’m not sure what this means.

As I said, I’ve already changed the database prefixes to lower case. In fact, when I changed the wp_config to lower case, it locked me out.
The error message is coming up as FPB in all caps, but that’s not what I’m seeing on the backend. What backend I can get to, at any rate, because I’m locked out.
This is on localhost, also.
