My forms are dead simple as that post suggests and they still do not work correctly. As of today, neither I nor the user gets an email after the form submits. Like gemba, I did not change anything before the form stopped working.

buzztone – Thank you so much for the link. It was most certainly my web host, Dreamhost, which does not allow a “From” address that is a non-Dreamhost address.
Thank you for your response Takayuki. I do receive the “Your message was sent successfully” response every time I submit the form. I’ve also tried a Yahoo email address, a personal Gmail address, and a business email address. None of them receive the email and I don’t see the messages in Spam either.
