I noticed this same thing and thought it was an issue in the code until I simply logged out of my account and went to the page as a customer. I think that the plugin pulls your default website email and auto-populates the form. You can also try it on a phone or pad that isn’t logged in. I have Kalium theme though. Hope this helps.
