This was a fresh install of WP as I have never used it on this domain before via my web hosting server.
Therefore, I would assume the issue is more likely #3 or 5 from your list.
I’ll reach out to my hosting provider to see if they can assist as I don’t have much a tech background to attempt anything and risk screwing something up on my end.
