Yes, it is running WP 3.1, with version 1.6.5.4 of the plugin.
I have tried it in Firefox 3.6.14 and IE 8.0.7600.16385
I did as you instructed and both checkboxes were already checked.
I went as far as uninstalling the plugin, deleting the plugin directory, then re-installing, but it still won’t appear 🙁
