    OK, to answer my own question.

    I have changed the plugin to be called with a shortcode rather than as a filter on ‘the_content’. The shortcodes do not get called multiple times when the other plugin (Yoast SEO) is installed, and hence my form only gets processed once.

    Hope that helps any others out there.
