The problem was never solved. I appreciate your time but this plugin doesn’t do much because it can’t read shipping prices from anything but a static field. For this to be useful, the plugin needs to support dynamic shipping options (ones that change based on shipping locations).
I have disabled every 3rd party plugin related to shipping and it still is not working:

Yes, this is of course how I have setup shipping, but without the WooCommerce Shipping option, nothing can be added besides a flat rate. Is adding flat rate shipping the only method that is supported by this plugin?

So how does the shipping cost work without using any plugins. Where would I input the shipping cost into a standard WordPress installation?
I use woocommerce. To have any shipping options you also have to use “WooCommerce Shipping & Tax” plug-in. Do you not support the usage of this plugin? And if not, how is it possible to calculate shipping using woocommerce?

I restored my site from a backup and reinstalled this plugin but just left the minify option disabled. This seems to work. I recommend you build functionality to enable the user to manually disable options from the command line. If I didn’t have my own backup of my website this plugin would have left me stranded with a broken site.
I cannot access WordPress admin, when I enabled minify, it broke my entire website. Nothing works. I cannot access any page on my website.
I cannot change settings from the GUI because when I enabled Minify it broke my entire site. Nothing works unless I manually delete the plugin. What file are the settings saved in so I can change them manually? Please read my original post.

Thank you for testing externally. I wonder if something on my internal network was preventing me from authenticating properly.

Thanks for your reply Dan, I checked and double checked that the only option selected is First-Class Package Service. Then I disabled the plugin and sure enough, the other options were still there. So not your problem!
What ended up fixing the issues was adding a shipping method to the “Locations not covered by your other zones”.
Edit: It turns out adding a shipping method to “Locations not covered by your other zones” just triggered WooCommerce to update. The real fix was disabling the WooCommerce Shipping & Tax plugin
