1. No, the drop-downs don’t appear after I paste in the code. They are always there.
2. The entire time there are no valid entries in the drop-downs.

Hi Robin, I followed those exact steps in the tutorial – I copied and pasted the code on the bottom right of the page for classic forms on the embedded forms page in MailChimp. I am still not seeing anything on the drop-downs for Name field and Email field.
