It was the Better WP Captcha plug-in that was causing it. Oddly enough, deactivating them one at a time (including that one) never fixed it. Turning them all on except that one keeps it fixed.

On the site where comments are working (Polarizing Images) the home page is set to display blog posts.
On the site where comments are not working, the home page is a static page and the blog posts appear on a page called The-Blog. This page has page comments set to ON and comments are enabled in the Discussion settings.

When, as logged in, I submit a comment, I do get the message “thanks for the comment.” The comment still does not show, however.

In reply to: WordPress 3.2.1 has broken the dashboardHey blahblahbecky, I emailed my host and they got it all resolved quickly (one more I reason I’ve been happy with them).
Fortunately, WordPress is not an uncommon platform so it is highly unlikely that you are their only client using WordPress. That means that you should not be the only person experiencing this issue so, as blipfish mentioned, go ahead and email them. They’ll probably respond pretty quickly.

Never mind, I located the (likely) problem – my host is running PHP 5.2.16

In reply to: Restricting CommentsI had that ability but now it has disappeared. I can either turn comments on or off globally but not for either specific posts or pages. It happened the same time that the drag-and-drop for widgets stopped working.
I “fixed” that by enabling accessibility but I really need to know how to fix turning off comments for specific pages.
It started when I upgraded to WP 3.1 a few days ago.

I just started encountering this issue, too.
I updated the theme, am running the latest version of WordPress and the form plug-in.
I disabled all the plug-ins and I still have a duplicate form. If I remove the short code from the contact page, both forms go away.

My wife is using the form on her blog (http://rachel.domaschuk.com/wordpress/?page_id=28) and also is experiencing the problem.
