I created a stage site as advised and the form now is loading fine, both in desktops & mobiles.
I didn’t need to deactivate & reactivate any plugin.
Do you think that might be a theme or server issue on my live website?

It actually happens to all my forms so I send you just one of them.
In Firefox works with no problem.
In all other browsers it comes a red error with no text. I activate & then again deactivate the WP-Optimize plugin and the forms work fine for a few hours. After a while again the same red error.
