Just wanted to say that I installed this tonight and so far it seems to be working well with my tests. Sean, thank you so much for putting it together; it is a major improvement over the standard WP email.
I have this problem as well. It deactivates email notifications on the topics that required moderation. If anyone has a solution, I would be interested in it. I have deactivated the plugin for now, but I liked the plugin and would like to continue to use it if the email notification issue could be worked out.

I’m also having this problem — email says: “Password: both” and the password doesn’t work to log in.
I’m trying to use integrated with bbpress. The bbpress registration page requires username and user email.
Any tips?
