Same exact issue here. The form shows twice. Once where it is supposed to be and then once again above it. If i deactivate the plugin the form returns back to normal but does not do it job properly (never sends to mail chimp).

Thank you! I will ask my host (since this host sucks and i see no way to do it myself) to turn on OpenSLL. Hopefully that works.
