PHP mode should “just work”. Check the source HTML returned to the client for the tag appended by supercache to make sure it’s serving the cached file.
Using the equivalent of mod_rewrite requires using URLRewriting eg: http://www.freescape.com.au/supercharging-wordpress-on-iis/
Setting this up will cut php out of the picture entirely.
