Thanks for tackling this @lukecarbis. It was actually a co-worker of mine who submitted the report on the Github. We figured we would post in both areas to increase the visibility for other users who might be running into the same issue.

The issue here was with the original purchase of the Gift Card. The Gift Card was purchased with a coupon code that allowed the user to get a free gift card.
When the user purchased the gift card there was NO error message or warning telling him that using a coupon code was not allowed.
Can that be fixed?
Pretty sure the following URL setup will work:
http://www.website.example/checkout/?add-to-cart=4843&gift_amounts=10
Where 4843 is ID of product and 10 is the amount.
