I have exactly the same problem after migrating a website. Controlled php, mysql, added new administrator, removed NextGen Gallery and made a new, fresh install. However, the error message still appears and I don’t know anymore how to solve it. Did you find a solution already?

In reply to: Media library doesn't show the imageThe thumbnail images in my Media Library don’t show up either in WP 4.2.3, they stay blank. Only in Edit I can see them. If I ad them to the website, I don’t get to see anything either.
See: http://screencast.com/t/q6g0byhBeI controlled the Upload folder with FTP, everything is there and correct. I controlled htaccess file: Nothing wrong. I controlled WP Media settings: Nothing wrong. I tried 4 different themes: the problem stays, so it is not in the theme.
I installed NextGen Gallery to see if the problem would exist there too: No!, nothing wrong. All images show up fine.If anybody knows where the problem could be situated, please let me know. There will be more than 1 user on this website and I am sure many will use Media Library, so this has to be solved.

In reply to: [Polylang] Clicking flag opens frame instead of pageThanks Chrystl, you remark brought me on the right trace. Indeed it is a conflict between NextGen Pro and Polylang. Do, i know now where the problem is situated. However i don’t know how to solve it.
Maybe adding some code?
Who knows how to solve this? I appreciate your help
