I am experiencing this exact same problem as kcpearson after upgrading from a very old version (2.5?) to 3.4.2. The upgrading was manual and I had to upload each of the hundreds of files over several hours – its possible that I missed a file or two – could that be the cause of this?
I don’t think I have a plig-ins problem as the problem still persists afer deactiving all plugins. I also tried different themes – again no luck! Anyone have any clues?
