I’ve seen the same issue.
I’ve tried updating & disabling all plug-ins and checked permissions but nothing and no help offered either. It’s ridiculous.
@andrew, I was thinking that by running through various issues in public, we might be able to solve both our issues and those of mmcneely. I couldn’t see the logic in creating yet another thread to discuss *exactly* the same issue. But I’ve been told, so I’ll shut up.

I’ve just tried disabling all plug-ins and still cannot add images. I also tried changing to twentyfourteen while the plugins were disabled. No joy. Help?
