Problem solved. ignore this issue.
The mosaic display tipped it off. I deleted the 3 reviews that represented that gap and everything is working fine.

Also noticed on the form page for folks to submit, a discrepancy in the way the mosaic display is working – bit gap in left column. That space seems to represent 3 testimonials.
