But,
I took a big scare, because WordPress lost permalinks cache (in the production version :S)I fixed it by selecting another option permalinks and then back to my permalinks option.

I have the same problem. The blank page problem appears with both Google Forms and WordPress SEO activated at the same time. The problem goes away if I disable either of them.
The error are with WordPress 3.8.3 and WordPress 3.9.
I have installed Google Forms 0.65 and WordPress SEO 1.5.2.7I have a WordPress MU. In one work without problems and another have blank page problem.
