Actually, upon further review. Issue # 2 is NOT resolved!

It is referencing a post that does not exist. I was trying to add an image to an earlier post: http://www.collegelax.us/wordpress/2007/09/17/ohio-state-mens-lacrosse-looks-to-set-attendance-record-next-spring/

Issue 2 is resolved. User error on trying to upload images and post a new WordPress post at the same time. Upload the image first, then develop the post.
I will try your suggestion for # 1 and # 3.
