The other thing is I chose to not have an image next to the form, but the form won’t center. Can you let me know how to fix that as well?

I’ve done exactly that, multiple times. When I have the multiple level donations and choose “yes” to custom amount, all that comes up are the standard level donation amounts.
I meant have both a multi level donation and a custom amount option at the same time. I want to have four standard options plus a custom amount as the 5th option. Is that possible?
