There is no setting in UM for that, but that’s exactly how it’s displayed in mobile devices…
Take a good look at each row containing the fields in Admin => UM => Forms. There should be 3 tabs to choose from for 1, 2 or 3 columns…
If you are installing a fresh WordPress site with nothing else but Ultimate Member 1.3.77 and continue to have that issue, then I would do another test but with WordPress 4.6 and Ultimate Member 1.3.72.
If you are still having that problem, then it’s server related because I know for a fact that I don’t get that problem. And, if doing that solves the problem, then you just discovered a bug in 1.3.77 and should be reported here:

Did you check Ultimate Member set up as I advised you? Otherwise, what exactly did you do?
That code above is useless. @mmak0710, you were already given a url to find this type of information. Results are not going to be exact so you have to modify code bit. Please stop posting customization questions on this free UM support board.
Dropdown menus.
Sometimes is because you changed something. For instance go to your Admin panel (left side menu) > Ultimate Member > Settings > Set Up. Make sure UM profile page is selected properly with the pulldown menu (e.g. User page: User).

Situation 2 : post & pages are indeed controlled by the UM access control widget, including the category they belong to. Setting : “Content accessible to Logged In Users”.
Despite this setting, the member-only content is displayed in the search results and visible to not-logged-in users. These results should be hidden.Not once did you mention the access control widget and you expect one to understand what you are doing and give you support?
The only real question left to answer is why is this happening to you. I suspect the usual culprits; the theme itself. If that’s not it, then one of your plugins…

Situation #1 behaves that way because you selected ‘logged in users’ in General settings.
Situation #2 probably behaves that way because your posts or pages are not being controlled by the UM access control widget which should be ON and ticked to “Content accessible to Logged In Users”.
Confirm you did that.
Nobody would know that. Rollback to prior to installing those two plugins.
I said check the “page” not user roles.

You should stop elbow bending while working on your site.
That page is probably not allowing access to logged in users. Check all your settings again.
Are you trying to change password as admin or user?
When insert new password and password confirmation in reset password page…
did you click on the email reset link first?
