Theme on this site is Traveler By Shinetheme, and there are lots of plugins in use, including Woocommerce, Advanced custom fields, Yoast SEO, etc., but none that seem to impact contact forms or this page in particular.

Thank you for your suggestions. I’ve done a couple of tests, turns out the problem was that we had the same accordion pasted 2 times on one page (one for desktop design, one for mobile design). I created a separate accordion (with a different ID) for mobile version which solved the problem.
