I’ve made a new page using the exact template that is used for the Home page and the form works correctly there:

It might be something to do with it being on the root index page, rather than a plugin clash

The Uploads directory and contained directories have permissions of 755 and mc4wp-debug.log has permission of 644 so it should be writable
I disabled logging in functions.php anyway and the problem still persists
I noticed since that the issue only occurs on the home page (even if i strip out all content besides the header and footer), on other pages it still works as expected
