Yoast Plugin image issue is still a problem? Crawl issues

Hi. The error that was created with the incorrect coding for images is still rearing it’s head for me and I’m not sure why. I just logged into my Webmasters Tools and there are again 64 crawl errors – all image issues like the indexed link attached. Why is this still happening? it’s been fine for ages. I have the other plugin that Yoast supplied to have the images de-indexed and I have the newest version of the plugin. Help? I’m confused. This error has cost me so much time and traffic to my sites over the past 12 months.
