If database permissions were the issue, I wouldn’t be able to update a page on one of the sub-sites (which I was able to do this morning). All normal site functions work, it just seems like adding a new site hits a wall.

It creates the entry into the “Sites < Network Admin” screen, but nothing beyond that.

Last site was added the day before 3.1 was released.

Slightly incorrect. Adding a PAGE is adding an entry to a table. Adding a SITE is creating new tables. So. Please check the database permissions for FULL perms. It’s possible things got borked.
Failing that, you know the dance. Turn off all your plugins and try to create a site.

Under “Must-Have” I have”Wordpress MU Domain Mapping” and under “Drop-ins” I have “sunrise.php”. I’m removed each of them and still not working.
I also noticed that the last successful blog number was 9, but my latest test was number 20. Even tried with different names, addresses & emails. As before, it saves to the blogs table, but does not create the tables needed for the rest of the install, which results in no dashboard to edit. The privileges have been reloaded and are identical to the other server running WPMU. The only “Settings” when you edit the “Site” is “Site Upload Space Quota”.
Have you talked to your host?
Right then. If it’s not making the tables, either (a) your ID doesn’t have the permissions or (b) a plugin is screwing it up or (c) your host is screwing it up.
It’s not B since you turned them ALL off. So A and C mean talk to the host :/

Problem with C is, I am the host (my server). :\ I’ll keep looking. It’d be easier if I was getting some sort of logged or displayed error message.

Anyone know where (which file) the SQL queries are sent when the sites are added?
same as when you install WP. Look in wp-admin/includes/schema.php & upgrades.php
I am having exactly the same problem. It should not be a db or host problem because it works fine in other domains at the same host. Please let me know if you find a solution.
I have determined that my problem was being caused by plugin Weg Traffic Genius.
