thanks for your question.
This is quite easy: Either simply add that heading data in the CSV file (or the program that generates the CSV file), or (which likely is what you need now) just insert a new row before the first one. To do that, simply check the checkbox for the first row and click the “Insert” button below the table text fields. After that, you will have an (initially empty) new row, which you can fill with the headings that you want.

Thanks for the quick response, Tobias.
I tried inserting a row but nothing happens. Here is what I did: (1) I checked the box to the left of row 1. (2)I scrolled to the bottom of the table (more than 400 rows)to the ‘Data Manipulation’ box. (3)Next to ‘Selected rows:’ I checked the’Insert Row’ button. The progress indicator works and the page resets to the top of the table, but nothing changes.
Addionally, I tried to ‘uncheck’ the ‘The first row of your table is the table head (HTML tags <thead> and <th>).” box. When I click on ‘Update changes’ the page appears to save, but the check mark reappears.
Any ideas?

Additional info, Tobias. I just tried to add a row to a much smaller table and it worked (I should have tried that before I sent you the last post). Is there a limit on the number of rows I can use?

that’s actually the correct procedure.
Now, your description however (and unfortunately) hints that you might be affected by the “Suhosin” problem, see for example http://wordpress.org/support/topic/plugin-wp-table-reloaded-long-csv-file-import-with-578-rows
This prohibits editing of large tables 🙁

I used your first suggestion and modified the csv file which took care of the heading issue. Right now I have a table with more than 1500 items that displays fine on my site.
