• I was having trouble with email forms in general with streamline.net. But the contact form I had in wordpress, even though it says its been sent no email turned up.
    I eventually worked out that streamline.net will only send a form to email with 3 fields, subject, email and message. So you set your form to have extra fields within the message part. Once this is done copy and paste the code directly into your wordpress page, code below, I just made up the content to check it worked!

    paste this into your page: the very top line will need changing to the path of the mail.php, which you need to ftp separately to your site (mail.php at bottom of page)

    this seems to work, all you need to do is change the fields to whatever you neeb
