Regarding to unique form ID. You should define it directly in the shortcode as described in https://wordpress.org/plugins/wcp-contact-form/faq/ – “How can I add a form to a page” section. You can add any value as unique ID – samples you can find https://wordpress.org/plugins/wcp-contact-form/faq/ – “Shortcode examples” section and https://wordpress.org/plugins/wcp-contact-form/screenshots/ – screenshot #4 – “Form Shortcode”.
Regarding to “Entries”, this is menu item, where you can find received messages, as described in https://wordpress.org/plugins/wcp-contact-form/faq/ – “Where can I find received messages” and https://wordpress.org/plugins/wcp-contact-form/screenshots/ – screenshot #6 – Entries

We added useful hints in the settings of the contact form, hot-button for shortcode adding in the TinyMCE toolbar and other features in the new plugin release. Also we reorganized and improved plugin documentation for better understanding (Description/Installation/FAQ).
