Wanted: Poll/Survey Plugin, Recommendations anyone?

Looking for a poll or survey plugin that I can have pop open or popup in a the style similar to a lightbox when a visitor enters the site asking them a couple of brief questions. And then asks them a different set of questions when they attempt to exit the site.
