Yes, there is a known plugin incompatibility when using w3tc with active CDN. I opened a ticket here: http://wordpress.org/support/topic/plugin-conflict-with-leaflet-maps-marker-cdnjs-issue?replies=2
Unfortunately there hasnt been a fix yet, though there is a workaround: navigate to w3tc settings page / CDN / Advanced and add the following to the “rejected file”-list:
/wp-content/uploads/leaflet-maps-marker-icons/*
Hope, that w3tc devs solve this soon, as I dont see a way to fix this on my side.

i am not using CDN, but turning off minify the map appeared. I ll investigate more

plugin which minify javascript often cause issues with my plugin. Will check that too and if I cant solve this (which I fear), I will enhance the compatibility checks to also show an admin notice when w3tc with minify option is enabled…

I found a solution: please navigate to Performance / Minify / Advanced and add the following to “Never minify the following JS files”:
wp-content/plugins/leaflet-maps-marker/leaflet-dist/leaflet.js
Will add a compatibility check for the next release for this.
